Our Disability Support Services

Affinity Disability Services offers a range of in home and community supports for people with a disability. Our support empowers participants and builds capacity for people with a disability. See below for what supports are available.

  • Support Coordinators help to better understand NDIS and all that comes with accessing this support.

    A support coordinator will link you with services, strengthen your network and ensure that you receive quality supports and services to help you achieve your goals.

    We currently provide both Level 2 and Level 3 Support Coordination.

  • A Psychosocial Recovery Coach is a professional who helps people diagnosed with a psychosocial disability.

    Our Coaches offer a hands on approach using their knowledge of mental health services to ensure that you receive the best support to reach your goals. We will work with you to design, plan and implement a recovery plan tailored to you and your goals.

  • Our support workers can assist you to complete every day tasks such as self care, cooking, meal prep, yard and garden maintenance, household cleaning, organisation and more.

    Our team are dedicated to increasing your capacity to complete these tasks independently and develop skills in areas where you eel you require improvement and want to learn more.

  • Our team supports participants to engage in activities they enjoy, explore new activities and getting everyday tasks done. There is no limit to what social and community access can look like when accessing support with us, however here are a few ideas: shopping, appointments, transport to see family and friends, going to events, bowling, fishing AND MORE.

    Our team are open, friendly and non-judgmental. They provide a safe space for you to be who you are and do the things you love.

  • Affinity is able to offer a range of supports in different accomodation options. Whether you are living in Supported Independent Living (SIL), Individualised Living Options (ILO) or are living in your own accommodation. We are happy to tailor a package to meet your needs. Refer to our accommodation page for more information on different packages available.

  • Affinity is a registered provider for Short Term Accommodation. The way we deliver this support is through working with you to find the best available option to meet the needs you have and tailor a package to meet those needs. We aim to ensure that during your Short Term Accommodation you are building skills and capacity, while being able to take a break from your current environment.   • We have a number of programs that we run through Affinity which focus on skill develop and building independence. We have supports available including Cooking Groups, Arts Groups, Budgeting support AND MORE. To find out more information see our “Groups” page or contact us.
